This illustration by Henry Justice Ford is a classic when it comes to depictions of Tales of King Arthur. He was a prolific and successful English artist and illustrator, active from 1886 through to the late 1920s. This illustration was first used in "The Book of Romance", published in 1902. However, this particular page is most likely from a reprint based on this book a few years later and not from the original 1902 edition.
